Protocol and limitations — V1.9
Validation status: Automated boundary, safety-gate, and dose arithmetic tests passed in a simulated browser DOM. This is NOT clinical validation. A clinician/pharmacist must review the exact protocol, every rule, and the real browser UI before patient use.
Hospital formulary: 2, 3 and 5 mg tablets, each splittable into halves. Scheduler prioritizes selected existing strengths when requested, then weekly total, daily consistency, and tablet simplicity; it does not store patient data.
Default: UW Medicine Simplified Nomogram for Warfarin Maintenance Dosing. This differs from the UW Health / ACCP-style nomogram reproduced in the 2013 review. Do not combine their INR cutoffs or hold instructions.
For goal INR 2–3: <2 increase 5–15% (booster only by clinician); 2–3 unchanged; 3.1–3.5 decrease 0–15%; 3.6–4.0 consider holding 0–1 dose and decrease 5–15%; >4 hold until therapeutic with clinician assessment. For goal INR 2.5–3.5: <2.5 increase 5–15%; 2.5–3.5 unchanged; 3.6–4.0 decrease 0–15%; 4.1–4.5 consider holding 0–1 dose and decrease 5–15%; >4.5 manual management.
Safety: Routine maintenance planning only for stable, nonbleeding adults. Critical or complex cases require individualized management. This prototype does not recommend vitamin K, bridging, or reversal. Follow-up windows are illustrative and must be reviewed against the chosen source guideline and clinical context. Any temporary hold is a separate clinician decision, not embedded in recurring 7-day maintenance doses.
